​Eligibility Requirements

• Ages: 18-60

• State of residence is not AK, CA, CT, DC, HI, NH or NY

• Actively at Work 30 hours per week and not disabled

• Occupation must be insurable (ask)

• Must apply for Illinois Mutual's Disability flagship Disability Income product, policy form DI105, and meet basic underwriting guidelines (ask).  However, it is not necessary to be approved for, accept, or maintain the companion policy.
